Job Description

Job Summary: The Health Services Nurse is responsible for providing RN oversight for health services across day and residential programs. This role involves developing, reviewing, and approving Health Care Plans, Plans of Care for Proxy Caregiving, and Health Risk Screening Tool (HRST) assessments. The Nurse will also oversee and ensure the provision of all medical-related training, including Proxy Caregiver training, medication administration, and emergency procedures. This position will develop and implement Medicaid Waiver Funded RN services, ensuring all practices align with DBHDD requirements and Person Centered Planning principles.

Key Responsibilities:

  • Clinical Oversight:
    • Supervise LPNs and non-licensed staff.
    • Ensure compliance with nursing practices and regulatory requirements.
    • Conduct regular audits to ensure proper medication management, including storage, security, and administration.
  • Policy and Procedure Development:
    • Assist in the development and approval of policies and procedures related to health and nursing services.
    • Implement Medicaid Waiver funded RN and LPN services in accordance with DBHDD and DCH guidelines.
  • Medication Management:
    • Ensure compliance with all state and federal medication management laws.
    • Review and audit controlled substance logs for accuracy.
    • Oversee medication administration, including assisting individuals with self-administration.
    • Ensure new prescriptions are implemented within 24 hours and conduct monthly medication management audits.
  • Training Provision:
    • Develop and oversee medical-related training, including Proxy Caregiver training, medication administration, and emergency procedures.
    • Ensure all training aligns with Person Centered Planning principles and DBHDD requirements.
  • Health Risk Assessment:
    • Ensure timely completion and review of HRSTs, specifically for individuals with ratings of three (3) and above.
  • Direct Care Services:
    • Provide direct care and support to individuals, including responding to acute distress and performing health screenings.
    • Record and maintain accurate care documentation.
  • Documentation and Reporting:
    • Complete incident and death reports, and communicate serious medical issues to DQI.
    • Review and modify responses to critical medical incidents as needed.
  • Interdisciplinary Participation:
    • Participate in IDT and ISP meetings and make recommendations based on medical assessments.
    • Engage in the intake process and provide appropriate recommendations.

Qualifications:

  • Education: Associate’s Degree in Nursing; RN license in the state of Georgia.
  • Experience: 1-3 years of experience working with individuals with intellectual/developmental disabilities in medical or community settings preferred. 3-5 years of progressive leadership experience in Day & Residential services.
  • Skills: Proficient in Microsoft Office (Excel, PowerPoint) and various software programs. Strong communication, problem-solving, and organizational skills.

Requirements:

  • CPR/AED and First Aid certification, valid Georgia Driver's license. Must complete Transportation and Vehicle Emergency Evacuation training and De-escalation Techniques certification within 90 days of employment.
  • Ability to pass drug testing and criminal background screening.
  • Ability to travel 30%-50% of the time and lift up to 50 pounds.
  • Reliable transportation and ability to achieve an “Adequate Functional Health Literacy” score on the TOFHLA.

Physical Demands and Work Environment:

  • Work in various settings, including community outings and facility environments.
  • May require lifting, standing, and carrying up to 50 pounds, as well as assisting with personal care needs.
  • Frequent travel within designated territory (30%-50% of the time). Must have proper hearing, vision, and speech capabilities.
